I’m having issues with a pretty standard CAS proxy setup that was working at SP2 RU6 until environment was upgraded to SP3.
Logon doesn’t fail every time, but does fail around 60% of the time and has a slightly better percentage when choosing “private” as the OWA client option. Whether using WinXp with IE8 or Win8 with IE10, the same thing happens.
When logon fails, a blank OWA logon screen is redisplayed and after a second logon attempt the URL is suffixed with /owa/ping.owa and a blank screen is displayed.
Application logs show two errors associated with failed logon attempts:
Front End CAS
Event ID 136
The sign-in to Outlook Web App failed. User <name> has a mailbox on 14.3.123.0 server version. However, no Client Access server or front-end server with a matching version was found to handle the request.
Back End CAS
Event ID 4999

Outlook 2007/2010, ActiveSync, and 2003URL redirection all work fine.
